This survey is for any students aged 5 - 11 who have been involved with a Partnership Grants project while attending a primary school from Year 1 - Year 6 (P1 - P6, or equivalent eligible years in a middle or transition school setting). It should be filled out at the end of the project.

If you need a teacher to help you with any of the questions, please do ask them.
